The Process at a Glance

Excluding the Assessment, we accept most major insurance plans.* 
Assistance maybe be available for certain individuals.

ALL Assessment's are Self-Pay only, and cannot be paid with insurance.

Co-Pay's may be required depending on your Health Care Plan. You (The Client) will be responsible for any and all outstanding balances.
Any unpaid balances may result in future treatment being denied and is grounds for termination from Treatment.

Rate Information

(*The Self-Pay rates listed are sliding-scale fee's which are only available to Clients who choose to self-pay)

Clients enrolled in the SOTP program will need to pay for Group Sessions and Educational Classes in addition to Individual therapy sessions. 
The Sexual Offense Risk Level Assessment (also known by its acronym S.O.R.L.) is also Required for ALL S.O.T.P. Clients.
The S.O.R.L. Assessment will also include an in-depth Mental Health & Substance abuse Assessment at no additional cost.

These rates are for Sliding Scale Self Pay Rates and are Subject to change without notice.

Your Financial Responsibility

By payment of the Assessment, you agree to pay the amounts listed either by Self-Pay or by Insurance prior to accepting service. You agree to resolve any outstanding balances with your treatment provider.

Failure to pay or resolve held balances may lead to an outstanding balance, which may result in treatment being refused and may have legal repercussions for court-ordered clients.

You agree to pay any insurance-related co-pay's due prior to receiving treatment.
